This form is managed by Safe Bars and is designed to provide a safe and confidential way for individuals to report violations of Lush Life's Code of Conduct policies while at Portland Cocktail Week. Please complete the questions to the best of your knowledge. Your safety and privacy are our top priorities. All fields are optional, and you have the option to remain anonymous. Please note that if you choose to remain anonymous, it may limit our ability to investigate or take direct action in some cases, but will still contribute to our efforts to create a safe and inclusive environment.
